Observation template

This template can be used by teachers in whole-group or small-group lessons or can be distributed to students to complete independently. It includes text boxes for recording observations for what they see, hear, feel, and smell. This template is commonly used in science classrooms, but can be adapted for all subject areas and grade levels.
